General Eligibility Requirements from the State of Georgia

All Applicants for the Certified Massage Therapist Program must meet and provide proof of the
following requirements:

1. Applicant must be at least eighteen (18) years of age; and
2. Applicant must have a high school diploma or its recognized equivalent; and
3. Applicant must be a citizen of the United States or a permanent resident of the United States; and
4. Applicant agrees to provide the Board with any and all information necessary, and
5. Authorizes the Board or its representatives to perform a criminal background check; and
6. Applicant must provide three (3) written references; and
7. Applicant must provide a passport photo (2” x 2”), taken within six (6) months of Initial Application.
